The Senior Vice President of Operations Transformation will lead the design, establishment, and expansion of Theoria Medical's comprehensive Central Operations hub. This role involves overseeing the development and deployment of specialized toolsets, automation platforms, and infrastructure to support a dual-revenue strategy: enhancing Fee-For-Service (FFS) revenue cycles and scaling Value-Based Care (VBC) workflows. Additionally, the role includes managing central utilities such as Medical Records, Laboratory Integrations, and Referral Management, while driving the implementation of operational strategies with facility partners. This position also provides secondary oversight for the daytime Telemedicine team to support facility launches and ensure broad coverage.

Key Responsibilities Central Operations & Ancillary Services Ownership

- Establish Centralized Hubs: Develop, staff, and scale a top-tier Central Operations function to standardize multi-state provider support, credentialing, and scheduling. - Medical Records Management: Centralize and automate the management of medical records to ensure compliance, rapid billing, and continuity of care. - Lab Workflows & Integrations: Create and oversee high-throughput pipelines for lab orders and results, ensuring timely delivery of critical diagnostic data to clinicians. - Referral Management Infrastructure: Design a centralized, seamless inbound and outbound referral management system to maximize patient volume and streamline care transitions. - Dual-Model Optimization: Create operational frameworks that boost FFS volume and capture while building workflows for VBC risk models. - Vendor & Tool Selection: Evaluate, purchase, and implement high-standard workflow automation tools to reduce manual workload across all centralized departments.

Facility Partner Implementation & Integration

- Onboarding Blueprint Strategy: Create and execute the operational playbook for the launch and integration within new partner facilities (SNFs, ALFs, and senior communities). - B2B Workflow Integration: Connect Central Operations with facility-specific workflows and systems (e.g., PointClickCare, MatrixCare) to ensure smooth clinical coordination. - Joint Governance & Relationship Management: Set up implementation timelines and joint operational steering committees with executive leadership at partner facilities to ensure accountability and alignment. - Change Management: Lead on-site training, adoption, and cultural change initiatives for facility staff to accelerate the use of Theoria's services.

Provider Productivity & Experience

- Maximize Clinical Throughput: Develop the necessary tools and systems to optimize daily clinician schedules, reduce travel times, and increase patient-facing hours. - EHR Optimization: Collaborate with product teams to streamline the proprietary EHR platform (ChartEasy), minimizing charting friction and preventing provider burnout. - Administrative Automation: Create and implement automated systems for administrative tasks, including documentation, lab tracking, and prior authorizations.

Value-Based Care Workflows

- Risk Model Tooling: Build operational tools that integrate VBC requirements directly into daily clinical workflows, focusing on preventive interventions and chronic care management. - Hospitalization Prevention: Develop real-time tracking systems within Central Operations to quickly identify at-risk patients and reduce hospital readmission rates. - Data Integration: Ensure seamless data exchange between Central Operations, remote patient monitoring (RPM) platforms, and facility partners.

Telemedicine Team Operations & Footprint Support

- Daytime Telemed Management: Optimize the daytime telemedicine team, focusing on maximizing clinician utilization and balancing cross-footprint coverage needs. - LEAD Launch Alignment: Coordinate telemedicine resources to support and stabilize new LEAD facility launches as a secondary operational layer.

Performance Management & Insights

- Own the Dashboard: Create executive-level productivity scorecards to monitor provider utilization, billing velocity, lab turnaround times, referral conversion rates, and value-based outcomes. - Enforce Accountability: Use analytics to identify underperforming workflows and drive swift process improvements across regional teams.

Qualifications

- Leadership Experience: 10+ years of operational leadership, particularly in building or scaling a Centralized Operations/MSO hub within a rapidly growing healthcare organization. - Ancillary Operations Expertise: Proven experience directly managing and scaling high-volume Medical Records (HIM), Lab routing systems, and Referral coordination departments. - Strategic Dual-Model Expertise: Extensive hands-on experience balancing Fee-For-Service operational metrics (billing velocity, coding accuracy) with Value-Based Care workflows (quality tracking, utilization management). - B2B Implementation Track Record: Proven success in leading complex, multi-site operational rollouts with external healthcare enterprise/facility partners. - Productivity-First Mindset: Proven experience in deploying technologies and tools that significantly increase clinical team capacity and reduce administrative burden. - Post-Acute Domain Knowledge: Deep familiarity with the post-acute care ecosystem, including skilled nursing facilities (SNFs), assisted living, and senior care networks. - Telehealth Management Experience: Experience in overseeing telemedicine operations or clinical coverage logistics (highly valued but secondary to central MSO operations). - Education: Bachelor's degree required; MBA, MHA, or equivalent advanced degree highly preferred. - Skills: Advanced financial modeling, risk-assessment frameworks, contract negotiation expertise, and proficiency with healthcare analytics platforms. Advanced Excel/PowerPoint skills required. Co-work experience preferred.
